Do we accept your insurance?
We are currently in-network with:
Anthem/BCBS, Cigna, ConnectiCare, United Healthcare, and Harvard Pilgrim Healthcare.
Nutrition services provided by a dietitian are not covered under Husky/Medicaid. If we are not participating with your insurance, we may be able to provide you with documentation to submit to your carrier for partial reimbursement.

Book our services with self-pay options
We offer a variety of self-pay nutrition services and package options. From initial consultations and follow-up visits for individuals or families, to grocery store tours and individualized family meal plans, we have what your family is looking for.
We require a credit/debit card to be placed on file upon booking an appointment.
Payment of all fees is expected at the time of service or via credit card on file. Rate quoted is for self-pay only. If your insurance carrier covers our nutrition services, we will bill carrier directly.
It is the patient/client’s responsibility to check insurance benefits and coverage. You will be responsible for any non-covered services, deductibles, co-payments or co-insurances, as determined by your insurance carrier. Accounts unpaid by the insurance carrier will be billed to the client.
Out-of-pocket payments can be made via credit/debit card, cash or check and are due on the date of your appointment. Credit/debit card payments can be made directly with your Registered Dietitian.
Please make checks payable to Balanced Nutrition LLC. There is a $35 fee for all returned checks.
Patient/client will be responsible to pay a $50 late cancel fee for any missed or cancelled visits, not made at least 24 hours in advance prior to the scheduled appointment time.
